How to Enter the Survey Sweepstakes
There are two ways to enter:
1. Online Survey Entry
- Visit the official survey website.
- Enter the store number and survey code from your receipt.
- Complete the customer satisfaction survey.
- Submit your contact information to receive one entry into the sweepstakes.
2. Mail-In Entry Method
Participants can also enter without making a purchase by mailing their personal details on a 3” x 5” card to the address listed in the official rules
Sweepstakes Entry Period
- The Habit Burger & Grill Survey Sweepstakes starts at 12:00:00 AM Pacific Time (PT) on Wednesday, September 10, 2025.
- The sweepstakes ends at 11:59:59 PM PT on Tuesday, September 8, 2026.
- The entire duration between these dates is called the Sweepstakes Period.
- Entries submitted before or after the official entry period will not be accepted.
- Participants must complete their survey entry within the valid sweepstakes dates to qualify.

The Habit Burger & Grill Survey Sweepstakes is divided into 13 different survey periods. Each survey period has its own entry dates, mail-in deadline, and winner drawing date. Entries must be submitted during the correct survey period to qualify for that drawing.

Odds of Winning
The chances of winning depend on the total number of eligible entries received during each survey period. Only one prize is allowed per person during the entire Sweepstakes Period.
