MyHabit Experience Survey Rules & Requirements

MyHabit Experience Survey Customers may enter the Sweepstakes to provide feedback on Habit Burger & Grill and win a prize. Eligible participants who complete the survey online or by mail can compete to win a year’s supply of free Charburgers.

Before entering, be sure to review the official regulations, which include who may enter and how it works. The contest is available to U.S. citizens aged 18 and up, and no purchase is necessary to participate or win.

What is the Habit Burger & Grill survey?

The Habit Burger & Grill Survey is an online customer satisfaction survey administered by Habit Burger & Grill to get input from consumers regarding their most recent eating experience.

Customers can complete the survey at MyHabitExperience.com by inputting information from their purchase receipt (or an alternative entry method). The poll includes questions regarding meal quality, service, cleanliness, and general satisfaction.

Eligibility Requirements

To enter the MyHabit Experience. To enter the Sweepstakes, you must be at least 18 years old and a legal resident of the 50 United States or Washington, DC.

The contest is invalid outside of the United States, including Puerto Rico, Guam, and the United States Virgin Islands. Employees of The Habit Restaurants, LLC, the Sweepstakes Administrator, their affiliates, and immediate family members are ineligible to enter.

Rules

  1. Open only to legal residents of the 50 United States and Washington, D.C.
  2. Must be 18 years of age or older at the time of entry.
  3. Void outside the U.S., including Puerto Rico, Guam, and the U.S. Virgin Islands, and where prohibited by law.
  4. Employees of The Habit Restaurants, LLC and US Sweepstakes & Fulfillment Company are not eligible.
  5. Employees of their subsidiaries, affiliates, and advertising or promotion agencies are not eligible.
  6. Immediate family members (spouse, parents, children, siblings, and their spouses) of such employees are not eligible.
  7. Individuals living in the same household as these employees are not eligible.
  8. Subject to all applicable federal, state, and local laws and regulations.

Requirements

  1. Must be a legal resident of the 50 United States or Washington, D.C.
  2. Must be at least 18 years old at the time of entry.
  3. Must follow all official sweepstakes rules and guidelines.
  4. Must not be an employee of the Sponsor, Sweepstakes Administrator, or their affiliates.
  5. Must not be an immediate family member or household member of ineligible employees.
  6. Entry must comply with all federal, state, and local laws and regulations.

Method #1: Using Your Receipt Online Survey

Step 1:
  • Make a purchase at a participating Habit Burger & Grill location.

  • Keep your receipt. Look at the bottom for the required “Receipt Information.” like this bellow screenshot

Step 4:
  • Enter the Receipt Information (Store Number: 9999 and 8-digit Survey Code: 5381 – 7638) exactly as printed on your receipt. like bellow
  • (If you enter it incorrectly, it may not be accepted.)

  • After filling in the receipt details, click on the Start button.
Step 6:
  • Complete the guest satisfaction survey.
  • Your survey answers do NOT affect your chances of winning.
Step 7:
  • After finishing the survey, fill out the entry form completely with your contact details.
Step 8:
  • Submit the form to receive one (1) Sweepstakes entry for that Survey Period.

Method #2: Enter by Mail (No Purchase Required)

You can enter without buying anything or completing the survey.

  • Take a 3” x 5” card.
Step 2:
  • Handwrite (print clearly) the following information:

  4. State
  5. Zip Code
  6. Daytime Phone Number
  7. Birth Date

  • Starts: September 10, 2025 (12:00 AM PT)
  • Ends: September 8, 2026 (11:59 PM PT)
  • Divided into 13 Survey Periods
  • One winner selected per Survey Period

Winner Selection

  • Winners selected via random drawing
  • Conducted by an independent Sweepstakes Administrator
  • Odds depend on the number of eligible entries received
